Holistic Family Practice VA ("Practice," "we," "us," or "our") is a Virginia-based private integrative, functional, and holistic medicine practice established in 2009. We operate a Virtual Global Holistic Healthcare Model delivering services including:

Clinical evaluation, diagnosis, and treatment services are provided to patients located in the United States in compliance with Virginia licensure requirements and applicable telehealth laws. A valid clinical relationship is established only through a formal intake process, clinician review, and mutual agreement to enter into a provider-patient relationship.
Our services — clinical and non-clinical — are not a substitute for emergency medical care. If you are experiencing a medical emergency, call your local emergency services immediately. Do not use our platforms, forms, or messaging systems to communicate medical emergencies.
Our holistic wellness coaching programs and non-clinical offerings do not constitute the practice of medicine, the rendering of medical diagnoses, or the prescribing of treatment. Participants in non-clinical programs acknowledge that they are engaging with health and wellness education and coaching, not clinical medical services, and that results are not guaranteed.
The Virtual Global Holistic Healthcare Model is a proprietary service delivery framework developed by Holistic Family Practice VA to extend integrative and holistic healthcare access to patients and wellness clients globally through secure digital platforms. Use of this model is subject to all terms herein and the following additional provisions:
The following constitute the exclusive intellectual property of Holistic Family Practice VA and its principal clinician, protected under the United States Copyright Act (17 U.S.C. § 101 et seq.), the Lanham Act (15 U.S.C. § 1051 et seq.), applicable trade secret law, and international intellectual property conventions including the Berne Convention and TRIPS Agreement:

You agree that any violation or threatened violation of these intellectual property terms would cause irreparable harm for which monetary damages would be an inadequate remedy, and that we are entitled to seek immediate injunctive relief in addition to all other remedies available at law or in equity.
You are granted a limited, personal, non-exclusive, non-transferable, revocable license to access and view our publicly available website content and any patient education materials provided to you in connection with your own care. This license is solely for your personal, non-commercial use and does not permit reproduction, distribution, or any use beyond personal reference. This license terminates immediately upon any breach of these Terms.
Medical records requests are governed by our Medical Records Request, Release & Case Summary Policy, Virginia Code § 32.1-127.1:03, and HIPAA. By submitting a records request form, you agree to the fee schedule, authorization terms, and fulfillment timeline set forth in that policy. Records are released only upon receipt of a complete, valid authorization and confirmed payment. The Practice's clinical summaries and case evaluations constitute professional clinical service fees and are not subject to Virginia's statutory per-page copy fee caps.
All service fees are disclosed prior to service delivery. Fees are subject to change with notice. Current fee schedules for medical records, clinical services, and programs are available upon request or at the point of service enrollment.
Payment is due at the time of service or upon invoice issuance unless otherwise agreed in writing. We accept credit/debit card, check (payable to Holistic Family Practice VA), and electronic payment methods including Venmo and Zelle upon request. Invoices are processed through GoHighLevel's secure payment platform.
Clinical appointment cancellations must be made at least 24 hours in advance. Late cancellations and no-shows may be subject to a cancellation fee. Digital programs and courses: refund eligibility is disclosed at the time of enrollment. Medical records fees are non-refundable once records preparation has commenced.
To the fullest extent permitted by applicable law, Holistic Family Practice VA and its principal clinician, staff, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services, website, or content — including but not limited to loss of data, loss of revenue, or personal injury — even if we have been advised of the possibility of such damages. Our total aggregate liability to you for any claim arising from these Terms or our services shall not exceed the total amount you paid to us in the 12 months preceding the claim.
You agree to indemnify, defend, and hold harmless Holistic Family Practice VA and its principal clinician, staff, agents, and successors from and against any claims, liabilities, damages, losses, and expenses — including reasonable attorneys' fees — arising from: (a) your use of our services or website; (b) your violation of these Terms; (c) your violation of any third-party rights including intellectual property rights; or (d) any false or inaccurate information you provide to us.
These Terms are governed by the laws of the Commonwealth of Virginia and the United States, without regard to conflict of law principles. Any dispute arising from these Terms or our services that cannot be resolved informally shall be submitted to binding arbitration in Alexandria, Virginia under the rules of the American Arbitration Association, except that either party may seek injunctive or other equitable relief in a court of competent jurisdiction to protect intellectual property rights or prevent irreparable harm. You waive any right to a jury trial and agree that disputes will be resolved on an individual basis, not as part of a class action.
Our services are operated from the United States. If you access our services from outside the United States, you do so at your own initiative and are responsible for compliance with applicable local laws. We make no representation that our services are appropriate or available in your jurisdiction. These Terms are governed by US law regardless of your location.
We reserve the right to modify these Terms at any time. Changes will be posted to this page with an updated effective date. Your continued use of our services after any modification constitutes your acceptance of the updated Terms. We recommend reviewing this page periodically.
If any provision of these Terms is found to be unenforceable, the remaining provisions will continue in full force and effect. These Terms, together with our Privacy Policy and any service-specific agreements, constitute the entire agreement between you and Holistic Family Practice VA with respect to your use of our services and supersede all prior agreements.
